Q.  How to obtain Community Support Services ??

A. 

1. Contact the Division of Developmental Disabilities(DDD) Social Worker to request Community Support Services (CSS). All adults who receive services from DDD have a caseworker already assigned to them and should contact that person.
   
2. The DDD Social Worker will then contact the provider, in this case, the J. Arthur Trudeau Memorial Center. The Case Manager will formulate a plan, in collaboration with the individual, for providing Community Support Services. .
   
3. The Trudeau Memorial Center staff submits the CSS plan to the Division’s Social Worker for approval and to determine funding level (the number of CSS hours per week for that individual). .
   
4. Once the funding level is determined and the plan has been approved, the Trudeau staff will begin providing Community Support Services.
