Day Habilitative Services
We offer Day Habilitative Services for individuals with
developmental disabilities over 18 years of age. Day Habilitative
Services encompasses a broad spectrum of services and supports that
include:
- Academic Teaching
- Adult Daily Living Skills Building
- Recreation And Leisure Programs
- Volunteer Placements
- Vocational Training
- Speech, Physical, And Occupational Therapy
The services
are provided from 8:45AM to 3:00PM, and part-time is an option.
In order to receive Day Habilitative Services, an individual must
meet the eligibility requirements of the Rhode Island Division of
Developmental Disabilities(DDD). All adults who receive services from DDD
should have a social worker already assigned to them and should
contact that person regarding eligibility issues.
For placement into the Trudeau Memorial Center’s Day Habilitative
Services, the DDD social worker would make a referral to our
Universal Case Management department. A team, typically consisting
of the individual, a family member, case manager, state social
worker, program staff, and possibly a nurse or psychologist, would
then be convened to evaluate need and appropriateness of placement.
Day Habilitative Services are primarily based at or out of:
Trudeau Patterson Building 250 Commonwealth
Avenue Warwick, Rhode Island
While the DDD funds the costs of the program, some small costs
may occur for certain activities which are paid for by the consumer.
Transportation may be provided by RIDE, the Trudeau Memorial Center,
the family or the residential provider.
